COMMUNICATION
· Please update emergency contact information whenever there is a change. Accurate Emergency Card information is essential to ensure the health and safety of your child at school
· Please inform the health office of your child’s health concern(s) since they can greatly impact his/her safety and academic success.
· Students should communicate illness or injury to school staff before contacting their parents during school hours.
· If you wish to have the school nurse communicate directly with the doctor regarding your child’s health at school, please complete, sign and bring in the authorization for release of information below.

HEALTH OFFICE VISITS
· Students must have a pass to see the nurse during class and passing periods
· Students may come to the health office during nutrition and lunch without a pass. Students are encouraged to come to the health office during those times for minor injuries and concerns.
· If your child complains of a non-specific illness, such as minor cold or stomach discomfort, please treat the symptoms at home and encourage him/her to stay at school the entire day.
· If your child begins to feel worse at school, he/she may visit the health office for evaluation. You will be contacted if the condition warrants it.

MEDICATIONS:
- PRESCRIPTION:
· All prescription medications to be taken at school (including inhalers & epi-pens) must have a U.S. doctor’s order and parent signature. A student may be allowed to carry the inhaler or epi-pen if the doctor and parent indicate it on the form and the school nurse has met with the student.
· A new form must be completed each year and on file the health office.
· Changes in medication require a new authorization form signed by the physician and parent.
· Medication must be in the original pharmacy-labeled container.
· Medication must be picked up from the health office at the end of the school year.
· Unclaimed medication will be destroyed at the close of the last day of school.

- OVER THE COUNTER (OTC):
· The following OTC medications will be provided at school by the school nurse beginning 2012-2013 school year:
Acetaminophen (Tylenol) or generic
Calcium Carbonate (Tums)
Cough drops or throat lozenges
Ibuprofen (Advil) or generic
· PARENT/GUARDIAN MUST SIGN AN AUTHORIZATION YEARLY GIVING PERMISSION FOR OTC MEDICATIONS TO BE GIVEN TO THE STUDENT. The form will be kept on file in the health office.
· With a signed permission, the medication(s) above and those designated and provided by parent may be given to the student for self-administration at the Nurse’s discretion or dispensed by designated personnel as delegated by the School Nurse.

PE PASSES:
- The school nurse can write a PE pass for a student with a note from the parent for up to 3 days.
- Provide the school nurse with a U.S. doctor’s order for a student’s P.E. excuse of a week or longer.
- Provide a written release from the doctor when your student is cleared to return to regular activity.

IMMUNIZATIONS:
Your child must have all the immunizations required by California in order to attend school. Please see resources below for more information on requirements and resources.

HEALTHY HABITS:
- Students should do the following to attain and maintain their optimum health:
- Eat breakfast and nutritious foods throughout the day
- Exercise outside of school regularly
- Get at least 8 hours of sleep every night – it is essential for brain growth and function.
- Wear glasses, contacts or hearing aids as ordered by the doctor.
